The office of sPorT plus physical therapy, LLC is located in Phillips, WI at 171 South Lake Ave. in the lower level of the Medford Co-op Building (the old Feed Mill building)  next to Cenex.

sPorT plus physical therapy, LLC is accepting new patients. Although not required by the state of Wisconsin, most insurance carriers require a prescription from your doctor before receiving any physical therapy services.

Tuesday and Thursday by appointment only.

Monday, Wednesday, and Friday to be determined after consult with physical therapist.
